Person-Centered Planning

methodology: person_centered_planning

By tailoring services to individual goals, strengths, and life aspirations through collaborative planning, the organization fosters autonomy, self-determination, and long-term community integration, because individual agency and dignity are foundational to sustained personal growth and meaningful outcomes. This strategy centers on treating each individual as the expert of their own life, using structured planning processes to co-create goals and supports that reflect personal preferences and aspirations. It differs from more standardized or system-driven models by prioritizing flexibility, ongoing adaptation, and the individual’s voice in all decisions—whether in education, employment, housing, or care—making it a foundational theory of action across multiple service domains.
