This strategy prioritizes the unique identity and choices of each individual as the foundation of care, distinguishing it from standardized or medically driven models. It manifests across settings—from hospice to residential care—through individualized care plans, family involvement, and attention to emotional, spiritual, and cultural needs, creating a shared theory of change that personalization fosters dignity and well-being in terminal illness.
