Accepts Medicare, Medicaid, private pay, private insurance, and long-term care insurance as valid sources of payment for nursing home and therapy services.

Operates a nursing home providing 24-hour nursing care to individuals requiring skilled nursing services, offering both long-term and short-term care without discrimination based on personal or financial characteristics. The facility has expanded over time to a total of 100 beds through phased additions supported by infrastructure improvements.

Performs screening assessments and PRI evaluations by nursing staff prior to admission to determine appropriateness for nursing home placement and to assess whether individuals can remain in the community with lesser care. Accepts qualifying individuals from home or community facilities.

Provides physical, occupational, and speech therapy services to inpatients and outpatient community members, including initial evaluations (one hour) and follow-up treatments (30–45 minutes), with physical therapy typically delivered three days per week and up to five days for special conditions. The Physical Therapy Department has been expanded to support these services.

Accepts memorial contributions in honor of deceased individuals, notifies families of such gifts, and sends a Card of Remembrance to designated individuals or families to acknowledge the donation.

Runs a non-residential adult day health program offering nursing services, exercise, meals, personal care, diabetic monitoring, and therapeutic activities for community members. This program was formally established as part of facility expansion efforts.

Offers individualized recreational programs based on resident interests and abilities, including movies, holiday parties, exercise groups, BINGO, arts and crafts, music entertainment, and religious services. Maintains a daily events calendar on each floor and keeps the activity room open daily.

Awards Employee of the Quarter four times per year to staff who exemplify resident- and family-centered care and recognizes outstanding contributions through honors such as the NYAHSA Employee of Distinction Award.

Uses memorial contributions and other donations to fund sensory and craft supplies, BINGO prizes, musicians and entertainment, holiday parties, and to maintain and update therapeutic exercise, ultrasound, and electrical stimulation equipment in the rehabilitation department.

Participates in the Centers for Medicare & Medicaid Services (CMS) Star Ratings system based on health inspections, staffing, and quality measures, and engages in the AAHSA Quality First initiative to improve aging services quality.

Offers beauty parlor and barber shop services to residents on a regular schedule (Tuesdays and Wednesdays) and maintains an outdoor courtyard with lighting, furniture, and flowers for resident and family use, accessible when weather permits.

Facilitates communication for residents through daily one-on-one room visits, video calls with family via FaceTime/Google Duo/Skype, and an email service that prints and delivers messages to residents with staff assistance for replies during weekdays.
