Integrated Social Determinants of Health

By integrating screening and response to social determinants of health into clinical and community care delivery, we improve health outcomes, because addressing non-medical barriers such as housing, food, and transportation enables sustained engagement and reduces systemic inequities in access and treatment adherence. This strategy centers on the belief that health cannot be improved in isolation from the structural conditions shaping daily life. It unites diverse organizations around a shared theory of change: clinical interventions are more effective when paired with targeted support for social needs. What distinguishes this approach from narrower service models is its systemic focus—embedding social care directly into care pathways rather than offering it as an add-on, thereby treating social determinants as co-equal with medical factors in achieving health equity.
