By combining social, health, nutritional, and recreational services into a coordinated program, we improve holistic well-being and engagement among seniors, because addressing multiple interconnected needs simultaneously reduces fragmentation and increases accessibility to care. This strategy centers on delivering a unified, multidimensional model of support for older adults through co-located or coordinated services that break down silos between domains like health, nutrition, transportation, and social engagement. Unlike narrowly focused interventions (e.g., meal delivery alone), this approach assumes that synergistic outcomes emerge when services are integrated, making participation easier and more effective. It is distinct from standalone or referral-based models by emphasizing intentional coordination within a single program or facility to holistically support aging in place and long-term well-being.
