Provides social services for senior citizens on Staten Island, including support programs and community engagement activities tailored to older adults.

Organizes an annual December gathering to commemorate individuals lost to AIDS, raise awareness, and celebrate advancements in medicine, policy, and client resilience.

Engages skilled volunteers for facility maintenance tasks such as painting, plumbing, and tiling, and accepts in-kind donations to support program operations.

Operates the Praxis Third Ave Shelter and Broadway Family Residence, providing emergency shelter for 58 adult families and families with children, prioritizing those from Brooklyn’s CB 6 and surrounding communities.

Supports the reunification of homeless individuals with their families, achieving 415 successful reunifications since 2006.

Connects clients to primary healthcare services and provides emergency medical care, food, and clothing upon admission. Has facilitated over 12,965 healthcare connections since 2006.

Provides traditional Thanksgiving meals to approximately 300 clients annually and hosted a 2021 Thanksgiving food drive in collaboration with East Coast Scooters. Also hosts an annual September picnic in a NYC public park for clients from all sites.

Facilitates permanent housing placements for individuals transitioning from homelessness, including apartments, supportive housing, and long-term congregate care. Has placed over 11,245 individuals in permanent housing since 2006.

Develops and manages a growing portfolio of supportive housing properties, including the acquisition of development sites and opening of permanent supportive housing projects such as Praxis White Plains Rd (2014) and Praxis Loring Place North (2017).

Provides case management, mental health evaluations, and individualized service plans to support clients in achieving housing stability and independent living. Clients meet weekly with case workers and supportive services staff to track progress toward personal goals.

Provides transitional and permanent supportive housing to formerly homeless individuals, including those living with HIV/AIDS, mental health challenges, substance use disorders, and those recently released from incarceration. Operates 345 transitional housing units and 102 scatter-site permanent supportive housing units in Brooklyn and the Bronx, serving over 1,100 individuals nightly and approximately 1,750 annually.

Employs individuals who have transitioned from homelessness to stable housing, supporting workforce reintegration for those with criminal or drug histories.

Offers more than 25 weekly workshops and support groups on harm reduction, treatment adherence, addiction counseling, recovery support, vocational training, and LGBT support to promote health and recovery among clients.

Operates the Bushwick Multi-Service Center in New York City, providing building space and managing delivery of comprehensive health and human services to a diverse population across the five boroughs. The center has been managed by the organization since 1986.

Provides direct advocacy to secure tailored residential and support services for adults with complex developmental needs. Also advocates for agency-wide social events such as galas to promote socialization, recognition, self-confidence, and community connection.

Merged with Queens Parent Resource Center in July 2016 to expand service capacity and geographic reach, enhancing organizational infrastructure and service delivery capabilities.

Manages client intake processes through the Director of Social Services and Community Engagement, ensuring structured and transparent information sharing with ongoing support. Conducts transfers, admissions, and discharges through senior administrative staff in compliance with all relevant laws and regulations.

Provides mandatory compliance training for all staff, conducts monthly exclusion checks and credential verification for licensed professionals, and performs regular internal audits and external evaluations to ensure regulatory compliance and program effectiveness.
