Home-Based Care Model

Provides home care, healthcare, and quality of life services to support residents of all ages in maintaining health and remaining at home in the Mid-Hudson Valley.

Provides specialized training for home health aides in disease-specific conditions, dementia, clinical care, and customer service; trains healthcare students and professionals; and hosts professional development seminars for regional health partners.

Provides personalized care navigation and life planning services for older adults and individuals with complex needs, including support for aging in place, enrollment in public benefits, coordination with community resources, and development of individualized wellness plans. Also offers care navigation services to employers to support employees during life transitions.

Provides skilled nursing, physical and occupational therapy, speech therapy, home health aide services, medical social work, and medical equipment and supplies to patients in Westchester, the Bronx, Dutchess, Putnam, and Rockland counties. Services are delivered in-home and include care coordination with physicians and other healthcare providers.

Funds and supports the sustainability of Visiting Nurse Services in Westchester, Inc.'s healthcare programs and services through resource development and charitable outreach.

Offers telehealth remote patient monitoring, cardiac care, pain and palliative care, mental health home care, fall prevention using interactive home-based technology, and community health initiatives such as influenza vaccinations and monthly health screenings for at-risk seniors at community sites.

Publishes annual reports and consolidated operations of affiliated entities into a single headquarters to improve organizational efficiency and transparency.

Conducts triennial Community Health Needs Assessments (CHNAs) using quantitative and qualitative data, stakeholder input, and public feedback, and develops CHNA Implementation Strategies and Community Service Plans to guide hospital and community benefit programming in the Capital Region.

Provides 3T MRI services at outpatient locations including the Walmart Plaza on Route 4 in East Greenbush and at St. Peter’s Hospital in Albany, enhancing patient access and comfort.

Offers same-day evaluation and diagnostic testing for non-life-threatening cardiac symptoms, including echocardiogram, electrocardiogram (EKG or ECG), and stress testing.

Offers multidisciplinary breast care through certified mammographers, radiologists, pathologists, oncologists, surgeons, nurses, and nurse navigators, with facilities accredited by the National Accreditation Program for Breast Centers and designated as a Breast Imaging Center of Excellence by the American College of Radiology in Albany and Troy.

Delivers accredited home infusion therapy services, administered in patients' homes and accredited by the Community Health Accreditation Partner (CHAP).

Prepares and delivers blizzard bags containing non-perishable food items to clients in the fall to ensure continued food access during winter weather-related service interruptions, in partnership with community groups such as the Leo Club, Teen Connect, and Emma S. Clark Memorial Library.

Utilizes volunteer nurses to conduct client intakes and assessments to determine eligibility and dietary needs for meal delivery services.

Conducts daily visual safety checks by requiring recipients to be present at delivery and having drivers confirm receipt, enabling early identification of health or safety concerns among homebound clients.

Provides home-delivered meals to homebound, disabled, chronically ill, or convalescent individuals who are unable to prepare their own food. Delivers two meals per day (a hot lunch and a cold evening meal) Monday through Friday to ensure nutritional support and safe food handling in compliance with Health Department regulations.

Offers need-based subsidies to assist clients with meal costs, ensuring no one is denied service due to inability to pay.

Engages office volunteers to support operations including route coordination, data entry, mailings, and delivery of diet slips to meal preparation sites, ensuring smooth administrative functioning.
