IN TOTAL, LSC RECEIVED AND SERVED 158 INDIVIDUALS WHO WERE REFERRED BY A DOCTOR, NURSE, OR SOCIAL WORKER (88 REFERRALS IN CALENDAR YEAR 2022).

- Host weekly virtual and in-person social clubs for older adultsOperates weekly virtual social clubs via phone or Zoom, facilitated by trained guides using storytelling prompts to foster connection, belonging, and purpose among older adults. Offers at least one in-person gathering per quarter to deepen relationships. Programs are available in English, Spanish, Mandarin, and Cantonese, serving over 2,500 older adults across all five boroughs of New York City.
- Run StoryRx program to connect isolated older adults with social clubsOperates the StoryRx program, which partners with healthcare providers and social service agencies to refer isolated, homebound, or chronically ill older adults to weekly social clubs.
