Provides behavioral health services to diverse populations, including adults, youth, veterans, active-duty members, and individuals across income levels, housing statuses, races, and languages.

Conducts Narcan training sessions that teach participants how to recognize and respond to opioid overdoses by administering Narcan. Distributes free Narcan kits to all participants completing the training, with certification provided upon completion.

Provides OASAS-licensed outpatient addiction treatment services for children, adolescents, adults, and family members, focusing on education, recovery, and relapse prevention. Operates a mobile substance abuse program offering free, tailored clinical treatment and mental health assessments for the LGBTQIA+ community, including in-home services. Deploys a mobile opioid support vehicle throughout Suffolk County to deliver free opioid recovery services, education, and harm reduction tools to adults and minors with parental consent.

Provides peer-led services to mental health clients through its Consumer Affairs division, where peer workers in recovery deliver support and share personal recovery experiences. Conducts peer outreach services to engage underserved populations and connect individuals to resources using lived experience as a foundation for trust and connection.

Enables clients to access their health records through the Foothold AWARDS patient portal to support informed decision-making and engagement in care. Ensures full compliance with ADA requirements in its 5310 Transportation Program.

Provides services to assist individuals in securing employment and building economic stability through financial management support. Offers residential programs for individuals discharged from psychiatric facilities that provide supervision and training in daily living skills such as meal preparation, budgeting, house cleaning, shopping, and socialization.

Operates a food pantry to provide food assistance to community members. Provides services to help individuals pay rent and maintain housing stability.

Operates two peer-supported, self-help community centers for adults with mental illness, offering socialization, meals, recreational activities, and peer support in a safe, informal setting. Provides one-on-one peer recovery support in inpatient psychiatric units to facilitate transition from hospital to community living. Offers the Respite Plus Program, which provides relief to caregivers through companionship-level care, home visits, community outings, and overnight stays at a respite apartment.

Delivers Assertive Community Treatment (ACT) to young adults aged 18–25 with persistent mental illness, including 24/7 support, rehabilitation, and case management. Operates a mobile Forensic Assertive Community Treatment (FACT) program for individuals with severe mental illness involved in the criminal justice system, including those under assisted outpatient treatment, experiencing homelessness, or needing vocational support.

Offers personalized care coordination services for adults with serious mental illness, chronic medical conditions, or substance use disorders, and for children and youth up to age 21 with serious emotional disturbance or complex trauma. Services include connecting clients to behavioral health specialists, primary care, housing, vocational support, and entitlements, with outreach within 24 hours and service initiation within five business days. Children’s services are funded through Medicaid and include referrals and ongoing coordination across treatment providers.

Provides crisis intervention services with access to a 24-hour emergency line and after-hours warm line. Offers intensive, person-centered residential transitional support services in Suffolk County with multidisciplinary teams including behavioral health workers, nurses, direct care staff, and peer specialists available 7 days a week and 24/7 on-call support.

Provides over 800 permanent, secure, and affordable housing units through the Supportive Scattered-Site Housing Program for individuals with disabilities. Operates multiple supportive housing developments including Herkimer Gardens (121 units for seniors in Central Brooklyn), the Barbara Faron Residence (118 units for low-income individuals in Far Rockaway), and transitional residential housing with 24/7 on-site support staff in Queens Village and Brentwood. Also operates the Irving Berkowitz Residence in Brentwood as a temporary housing program for individuals transitioning from acute hospitals to community living.

Provides respite care services offering temporary relief to caregivers by supporting seniors in their own homes. Offers senior companionship services including socialization, meal preparation, medication reminders, light household tasks, and accompaniment to outings for homebound or isolated seniors. Sponsors the Senior Companion Program in Suffolk County since 2002 and operates supportive housing for seniors such as Herkimer Gardens and the Barbara Faron Residence.

Operates a Companion Program that enables individuals in recovery to volunteer at community sites including hospitals, adult homes, food pantries, soup kitchens, and outreach programs, while receiving on-the-job training, peer support, and benefits such as meal and travel allowances. Has served as the sponsoring agency for the Foster Grandparent Program in Suffolk County since 1977, connecting low-income seniors with children needing support in schools and childcare centers.
